RACHEL STOLLEY

DNP
Hi, I am Rachel Stolley, and I have been an Advanced Practice Nurse since 2002.
My husband is on active duty Air Force, and we recently relocated to Colorado with our two boys. I grew up in a military family, so I’m not exactly sure where “home” is, but I’ve spent most of my adult life in Nevada and Colorado.
I graduated from the United States Air Force Academy and then attended Vanderbilt University in Nashville, TN, where I received my Master’s degree in nursing. I spent seven years serving in the Air Force as a Nurse Practitioner before transitioning to civilian practice.
I returned to school in 2012 and received a master’s degree from the University of South Alabama with a Family Nurse Practitioner (FNP) focus in 2014 and then graduated with a Doctor of Nursing Practice (DNP) degree in 2017. My doctoral focus was diabetes education because I have a passion for empowering patients with the knowledge needed to improve their health.
I became a Nurse Practitioner because I love developing relationships with my patients and working together to improve their health. My favorite part of being an NP is helping patients achieve better health and well-being via high-quality and compassionate care.

KATIE ADAMS

DNP, FNP-C
Special Interests: Preventative care Pediatrics Cardiovascular risk reduction
Hobbies: Baking, watching/going to the movies, crocheting, reading, and hiking
Favorite thing about primary care: I have worked in both adult and pediatric acute care settings and learned that there is some hospitalization that is preventable. My goal as a Nurse Practitioner is to provide care and education to prevent hospitalizations and improve overall well-being. I am able to achieve this by creating strong relationships with patients and working with them to create individualized treatment plans.

JAMIE HANOSH

ASHLEY VANDERJAGT

MA, LPC, NCC

From an early age, she was captivated by the wonders of the human body and the complexities of the mind. Growing up in the breathtaking landscapes of Colorado, she always knew her path would lead to a career in medicine. With unwavering determination, she earned her Bachelor’s degree in Integrative Physiology from the University of Colorado Boulder, laying the foundation for her journey.

Her pursuit of knowledge continued at the Red Rocks Physician Assistant Program in Lakewood, CO, where she not only obtained her degree but also pursued her Masters of Medical Science from St. Francis University. Over the course of seven enriching years, she delved into various specialties, from orthopedic surgery to inpatient neurology, each experience shaping her into the compassionate healthcare provider she is today.

Driven by a passion for hands-on care, she expanded her expertise by becoming certified in aesthetic injectables and therapeutic neurotoxins in June 2023. Currently, she’s actively engaged in a fellowship for Integrative Psychiatry and pursuing Certification in Brain Optimization & Restoration, eager to further her understanding and improve patient outcomes.

Through her own journey dealing with personal health issues, she has experienced first hand
the frustrations of not getting answers, being passed around different medical specialties, and
feeling hopeless in taking control of your own mind and body. It’s this personal experience that fuels her commitment to guiding others back to wellness, focusing on holistic approaches to symptom management and disease prevention.

Above all, her mission is simple: to help others unlock their full potential, both mentally and physically, and to be a beacon of hope and support on their healing journey.
